Are you getting requests from Ceridian for invoices to substantiate your medical reimbursements? If so, here is a tip for locating proof of all your medical charges through Anthem.
Register yourself on the Anthem website: www.anthem.com.
Click the tab “My Claims” that appears at the top of the page.
Search for your claim by any date, including all of 2008
A list of claims will appear, with the most recent listed first. If your claims overflow to another page, a list of pages will appear, allowing you to forward (1-5; 6-10, etc.)
Print the Details of the service date and provider you need.
Ceridian accepts these forms as proof of the service. If you have any questions, don’t hesitate to call Sheila for assistance.
