Anthem Open Enrollment

If you would like to make a change in your Anthem health coverage, now is the time to do it. Please call Sheila or Holly at the office to request an Anthem enrollment/change form. You may only make changes to your HEALTH insurance at this time. Open enrollment for all other policies will be at the first of next year.

In your packets this week, each full time Associate will receive a payroll deduction authorization form updated with our new health insurance rates. These rates become effective October 1st. Please put a check mark beside the coverage you currently have (if you are making no changes), authorizing the alteration in your payroll withholding.

All changes in enrollment and deduction authorization forms must be returned to the office by October 1st. If we have not received your deduction form, you will be kept at your current coverage level with the increase in rates taken out of your paycheck beginning October 1st, and we will follow up with you to have the form signed and returned.
